	MINIMUM 25 YEARS OF AGE
	1 YEAR VERIFIABLE OTR EXP WITH APPLICABLE EQUIPMENT

	NO RECKLESS, CARELESS DRIVING
	CLEAN MVR PAST 2 YEARS
